Replace FTD230 with LPC11U14
Also adds an LDO and xtal. Net BOM is about $0.50

Switch to on-board USB connector
This constructs a USB connector right on the PCB, saving the cost of
that connector, simplifying construction and giving back a lot of
board area.

Replace plated-through mounting holes with unplated vias
The oshpark boards I got didn't consistently fit over the mounting
pegs in the boxes. I'm hoping that the unplated holes will be more
consistent. And, if not, at least this board won't count on the ground
conduction through these holes and so I can get a 130mil drill and
clean them out.

Update resistor values to match what works. Add LED.
Testing revealed that the phototransistor appears to need quite a bit
of current to get the rise time within reasonable limits. Change the
resistor feeding it to 680 ohms (from 3.3k). This also required
changing the op-amp bias voltage from 1/2VCC to 90% VCC. Swapping in a
1k resistor for the 10k there fixed that nicely.
